Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#22-986-7 with Monument Impact, a non-profit organization, in an amount not to exceed $255,967, to provide
consultation, technical assistance and program support services to County’s Public Health Division’s Community
Wellness and Prevention Program (CW&PP), for the period from July 1, 2014 through June 30, 2015.
FISCAL IMPACT:
This Contract is funded 78% by California Department of Public Health; 12% by Metropolitan Transportation
Commission, City of Concord and City of San Pablo; and 10% b County Funds. (No rate increase)

BACKGROUND: (CONT'D)
County and its providers are meeting the requirements of the United States Department of Agriculture (USDA) and
the California Nutrition Network, as well as, supporting overall injury prevention, nutrition services and obesity
programs.
On September 10, 2013, the Board of Supervisors approved Contract #22-986-6 with Concord Community
Economic Development Organization, Inc. (now known as Monument Impact), for the period from July 1, 2013
through June 30, 2014 for the provision of consultation and technical assistance to the Department’s CW&PP with
regard to program compliance.
Due to extended budget negotiations, the Contract renewal was not processed in a timely manner. During that time,
Contractor continued to work in good faith to prevent a lapse in services to the community. Therefore, the Division is
requesting a retroactive start date of July 1, 2014.
Approval of Contract #22-986-7 will allow Monument Impact to continue to provide services through June 30, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, County will not have access to Contractor’s technical assistance and program support
services that ensure Compliance with USDA and State nutrition guidelines as well as Contractor’s expertise with
regard to injury and obesity prevention.
