Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #24-837-22 with Daniel May, M.D., a self-employed individual, effective October 1, 2014,
to amend Contract #24-837-21, to increase the payment limit by $35,840, from $174,720 to a new payment limit of
$210,560, with no change in the original term of May 1, 2014 through April 30, 2015.
FISCAL IMPACT:
This amendment is funded 100% by Mental Health Realignment. (No rate increase)
BACKGROUND:
For a number of years the County has contracted with Medical, Dental and Mental Health Specialists to provide
specialized professional services, which are not otherwise available.
On March 25, 2014, the Board of Supervisors approved Contract #24-837-21

BACKGROUND: (CONT'D)
with Daniel May, M.D. for the provision of professional psychiatric services to Adults in West County, for the period
from May 1, 2014 through April 30, 2015.
Approval of Contract Amendment Agreement #24-837-22 will allow the Contractor to provide an additional day of
psychiatric services, per week, to adults in West County through April 30, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, County’s clients will not have access to Contractor’s professional outpatient
psychiatric services resulting in a decrease in services to the community.
