HomeMy WebLinkAboutMINUTES - 10072014 - C.76RECOMMENDATION(S): Approve and authorize the Health Services Director, or designee, to execute, on behalf of the County, Contract #27-825-2 with Parham Gharagozlou, M.D., Inc., a professional corporation, in an amount not to exceed $300,000, to provide professional Primary Care and Sleep Study services to Contra Costa Health Plan (CCHP) members and County recipients for the period from November 1, 2014 through October 31, 2016. FISCAL IMPACT: This Contract is funded 100% by Contra Costa Health Plan Enterprise Funds III. (No rate increase) BACKGROUND: In April, 2013, the County Administrator approved and Purchasing Services Manager executed Contract #27-825-1 with Parham Gharagozlou, M.D., Inc., for the provision of professional Primary Care and Sleep Study services to CCHP members, for the period from November 1, 2012 through October 31, 2014. Approval of Contract #27-825-2 will allow Contractor to continue providing professional Primary Care and Sleep Study services through October 31, 2016.
