Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment/Extension Agreement #26-700-5 with Jaison James, M.D., a sole proprietor, effective July 31, 2014, to
amend Contract #26-700 (as amended by Amendment Agreements #26-700-1, #26-700-2, #26-700-3, and
#26-700-4), to increase the payment limit by $150,000, from $1,850,000 to a new payment limit of $2,000,000, and
extend the term from July 31, 2014 through September 30, 2014.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. (No rate increase)
BACKGROUND:
On August 16, 2011, the Board of Supervisors approved Contract #26-700 (as amended by Amendment Agreements
#26-700-1, #26-700-2, #26-700-3, and 26-700-4) with Jaison James, M.D., for the provision of professional
orthopedic services at Contra Costa Regional Medical Center (CCRMC), including but not limited to consultation,
training, on-call coverage and medical/surgical procedures, for the period from August 1, 2011 through July 31, 2014.

BACKGROUND: (CONT'D)
Due to lengthy contractual negotiations, the department was not able to process a renewal in a timely manner
therefore, the request is to extend and increase the term the payment limit to cover additional services through
September 30, 2014, while the agreement renewal negotiations are finalized. Approval of Contract
Amendment/Extension Agreement #26-700-5 will allow the Contractor to provide additional professional orthopedic
services at CCRMC through September 30, 2014.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, patients at CCRMC requiring professional orthopedic services will not have
access to Contractor’s services, which may result in a reduction in the overall levels of service to the community.
