Approve and authorize the Health Services Director, or designee, to execute, on behalf of the County, Contract
#27-757-3 with Armen Serebrakian,M.D. (dba Bay Area Hearing Services), a sole proprietor, in an amount not to
exceed $110,000, to provide professional audiology services to the Contra Costa Health Plan (CCHP) members and
County recipients for the period from September 1, 2014 through August 31, 2016.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan member services. (No rate increase)
BACKGROUND:
In December 2012, the County Administrator approved and Purchasing Services Manager executed Contract
#27-757-2 with Bay Area Hearing Services for the provision of professional audiology services, for the period from
September 1, 2012 through August 31, 2014.
Approval of Contract #27-757-3 will allow Contractor to continue providing professional audiology services through
August 31, 2016.
