Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#74-294-10 with Nadhan, Inc. (dba Creekside Rehabilitation & Behavioral Health), a corporation, in an amount not to
exceed $372,300, to provide Sub-Acute Care for Seriously and Persistently Mentally Ill Adults, for the period from
July 1, 2014 through June 30, 2015.
FISCAL IMPACT:
This Contract is funded 100% by Mental Health Realignment Funds. (Rate Increase)
BACKGROUND:
On June 4, 2013, the Board of Supervisors approved Contract #74-294-9 with Nadhan, Inc. (dba Creekside
Rehabilitation & Behavioral Health), for the period from July 1, 2013 through June 30, 2014, for the provision of
Sub-Acute Care for Seriously and Persistently Mentally Ill Adults.
Approval of Contract #74-294-10 will allow Contractor to continue providing Sub-Acute Care for Seriously and
Persistently Mentally Ill Adults through June 30, 2015.
