Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-684-3 with East Bay Medical Oncology/Hematology Associates, Inc., a corporation,
effective September 1, 2014, to amend Contract #26-684-1 (as amended by Amendment Agreement #26-684-2) to
increase the payment limit by $50,000 from $721,080 to a new payment limit of $771,080, with no change in the
original term of November 1, 2012 through October 31, 2015.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. (No rate increase)
BACKGROUND:
On December 4, 2012, the Board of Supervisors approved Contract #26-684-1 (as amended by Amendment
Agreement #26-684-2) with East Bay Medical Oncology/Hematology Associates, Inc., for the provision of
professional oncology services at Contra Costa Regional Medical and Health Centers (CCRMC), for the period from
November 1, 2012 through October 31, 2015.
At the County’s request, Contractor has agreed

BACKGROUND: (CONT'D)
to provide additional clinic services for the remainder of the contract term, due to an increase in utilization.
Approval of Contract Amendment Agreement #26-684-3 will allow the Contractor to provide additional professional
oncology services through October 31, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, patients requiring professional oncology services will not have access to
Contractor’s services which may result in a reduction in the overall level of services to the community.
