Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#74-225-5(7) with Laura Sousa, MFT, a self-employed individual, in an amount not to exceed $244,000, to provide
Medi-Cal specialty mental health services in West Contra Costa County for the period from July 1, 2014 through
June 30, 2016.
FISCAL IMPACT:
This Contract is funded 50% Federal Medi-Cal Funds and 50% State General Funds. (Rate increase)
BACKGROUND:
On August 21, 2012, the Board of Supervisors approved Contract #74-225-5(5) [as amended by Amendment
Agreement #74-225-5(6)] with Laura Sousa, MFT, for the provision of Medi-Cal specialty mental health services,
including treatment of Attention Deficit Hyperactivity Disorder (ADHD) and depression, for the period from July 1,
2012 through June 30, 2014.
Approval of Contract #74-225-5(7) will allow Contractor to continue providing Medi-Cal specialty mental health
services through June 30, 2016.

CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, patients in West Contra Costa County requiring Medi-Cal specialty mental health
services will not have access to Contractor’s services, which may result in a reduction in the overall levels of service
to the community.
