Approve and authorize the Health Services Director, or his designee, to accept, on behalf of the County, Grant Award
#28-862 (CFDA #14.703) from the Urban Habitat, to pay the County, an amount not to exceed $35,650, for the
County’s Public Health, Community and Wellness Prevention, Planning Institute project, for the period from April 1,
2014 through March 1, 2015.
FISCAL IMPACT:
Acceptance of this Grant Award will result in an amount not to exceed $35,650 from the Urban Habitat to fund the
Public Health, Community and Wellness Prevention, Planning Institute project. No County match required
BACKGROUND:
The County’s Public Health, Community Wellness and Prevention program (CW&PP) staff in collaboration with
other community based organization in urban planning have identified and confirmed organizations to host Contra
Costa County Planning Institutes workshops. CW&PP staff will work with planning institute host to develop curricula
for at least four (4) workshops within Contra Costa County focusing on organizations that have potential to influence,

BACKGROUND: (CONT'D)
and engage in local and regional planning decisions. The goal of the Planning Institute project will provide training
and educational strategies to shape economic opportunities for low-income communities and workers, tailored
curriculum based trainings on needs assessment, conduct session plans and create material to share information about
housing, transportation, and land use decisions within the community, and outreach to East County elected officials
in order to build relationship and promote a health in all policies approach.
Approval of Grant Award #28-862 will provide funding to support the County’s Public Health, Community and
Wellness Prevention, Planning Institute project, through March 1, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this award is not approved, the County will not receive funds to continue the testing support Planning Institute
project needs.
