Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Extension Agreement #26-699-3 with Semon Bader, M.D., a self-employed individual, to extend the term from July
31, 2014 through September 30, 2014, with no change in the original Payment Limit of $1,380,000, for orthopedic
services at Contra Costa Regional Medical Center and Contra Costa Health Centers.
FISCAL IMPACT:
None, there is no change in the Contract Payment Limit of $1,380,000. This Contract is funded 100% by Enterprise
Fund I. (No rate increase)
BACKGROUND:
On August 9, 2011, the Board of Supervisors approved Contract #26-699-3 (as amended by Amendment Agreement
#26-699-1), with Semon Bader, M.D., for the provision of professional orthopedic services at CCRMC including, but
not limited to consultation, training, on-call coverage, and medical/surgical procedures, for the period from August 1,
2011 through July 31, 2014.
Approval of Contract Extension Agreement #26-699-3 will allow the Contractor to continue providing professional
orthopedic services at
