Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-584-5 with Diablo Nephrology Medical Group, Inc., a corporation, effective August 1,
2014, to amend Contract #26-584-4, to increase the payment limit by $140,000, from $360,000 to a new payment
limit of $500,000, with no change in the original term of November 1, 2012 through October 31, 2015.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. (No rate increase)
BACKGROUND:
On December 11, 2012, the Board of Supervisors approved Contract #26-584-4 with Diablo Nephrology Medical
Group, Inc. for the provision of nephrology services, including, but not limited to, consultation, training and/or
medical procedures for inmates at the County’s Detention Facilities and for patients at Contra Costa Regional
Medical Center and Contra Costa Health Centers (CCRMC), for the period from November 1, 2012 through October
31, 2015.

CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, patients requirement these medical services at CCRMC or the Detention Facility
would experience much longer waits for services and may possibly go untreated.
