Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#22–970–12 with Center for Human Development, a non-profit corporation, in an amount not to exceed $606,790, to
provide health education and outreach services to residents of Contra Costa County, for the period from July 1, 2014
through June 30, 2015.
FISCAL IMPACT:
This Contract is funded by 19% Medi-Cal Administration Activities, 65% Enterprise Fund I and 9% CMS
Innovation Grant, 7% San Francisco Foundation Grant. (No rate increase)
BACKGROUND:
On September 10, 2013, the Board of Supervisors approved Contract #22–970–11 with Center for Human
Development for the period from July 1, 2013 through June 30, 2014, for the provision of health education and
outreach services to Spanish speaking residents of Contra Costa County.

BACKGROUND: (CONT'D)
Approval of Contract #22-970-12 will allow Contractor to continue providing services through June 30, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, the County will not be able to received health education and outreach services for
Spanish speaking residents of Contra Costa County from this Contractor.
CHILDREN'S IMPACT STATEMENT:
This program supports the following Board of Supervisors’ community outcomes: “Communities that are Safe and
Provide a High Quality of Life for Children and Families”. Expected program outcomes include an increase in
nutritious food and physician activity.
