1. CONSIDER waiving the 180-day “sit out period” for Howard Shaw, Family Nurse Practitioner in the Health
Services Department;
2. FIND that the appointment of Howard Shaw is necessary to fill a critically needed position;
3. APPROVE and AUTHORIZE the hiring of retiree Howard Shaw as a temporary County employee for the period
of July 30, 2014 through July 31, 2015, as recommended by the Health Services Department Director.
FISCAL IMPACT:
If the request is granted, there will continue to be salary and employment tax payment for the hours provided of
approximately $52,974. These costs will be offset by savings due to the vacancy of the permanent position.

Subject:Temporary Hire of County Retiree - Waiver of 180-day Sit-Out Period
BACKGROUND:
Howard Shaw has been employed as a Family Nurse Practitioner since May of 1995. He is an experienced
dermatology provider who currently works 22 hours a week of dermatology clinic and sees 29% of all patients in our
dermatology clinics. His retirement will cause a severe impact in access to dermatology services, with long waits for
care and delay in diagnosis of skin cancers and other conditions. Dermatologists are very difficult to find in the
current market. We currently rely on one board certified dermatologist and several experienced family physicians and
nurse practitioners, including Mr. Shaw. He has over 20 years of experience in dermatology and serves as a teacher
and consultant for other dermatologists and non-dermatologist providers.
Recruiting for a replacement provider with his training and experience will be extremely difficult. He is interested in
returning as a temporary retiree to work up to the allowed 900 hours a year in the dermatology clinic. He also has
been piloting a real time dermatology e-consultation system for primary care providers using secure photo messaging
within ccLink and he is needed to continue developing this pilot.
CONSEQUENCE OF NEGATIVE ACTION:
Failure to receive Board approval will result in significant negative impact in our ability to provide necessary
dermatological care services to our patients.
