Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-704-3 with Mt. Diablo Rheumatology Consultants, Inc., a corporation, effective July 1,
2014, to amend Contract #26-704-2, to increase the payment limit by $20,000, from $200,000 to a new payment limit
of $220,000, with no change in the original term of September 1, 2013 through August 31, 2014.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. (No rate increase)
BACKGROUND:
On September 17, 2013, the Board of Supervisors approved Contract #26-704-2 with Mt. Diablo Rheumatology
Consultants, Inc., for the provision of professional rheumatology services including clinic sessions, consultation,
training, and/or medical and surgical procedures at Contra Costa Regional Medical Center and Contra Costa Health
Centers (CCRMC), for the period from September 1, 2013 through August 31, 2014.
At the County’s request, the Contractor agreed to provide additional monthly rheumatology

BACKGROUND: (CONT'D)
clinics due to the departure of one rheumatologist formerly on staff at CCRMC.
Approval of Contract Amendment Agreement #26-704-3 will allow the Contractor to provide additional professional
rheumatology clinic services at CCRMC through August 31, 2014.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, patients requiring professional rheumatology services will not have access to
Contractor’s services, which may result in a reduction in the overall levels of service to the community.
