Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#26-771 with Health Leads USA, a nonprofit corporation to provide data sharing services for referral, outreach and
residential placement services to Health Center patients needing assistance to access non-medical community
resources, for the period from June 1, 2014 through May 31, 2016.
FISCAL IMPACT:
This Contract is a non-financial agreement. No County funds required.
BACKGROUND:
Under this Contract the Health Services Contra Costa Regional Medical Center and Contra Costa Health Centers in
collaboration with Health Leads USA will develop and track patients needing non-medical resources by
implementing a health leads desk at the West County Health Center for data gathering of patients needing assistance

This study is funded fully for two years through Kaiser Community Benefit Foundation.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, County and Health Leads USA will not participate in a collaborative data sharing
services.
