Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-883-23 with Rodney Chan, D.P.M., a self-employed individual, effective May 1, 2014,
to amend Contract #26-883-22, to increase the payment limit by $5,000, from $310,000 to a new payment limit of
$315,000, with no change in the original term of June 1, 2013 through May 31, 2015.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. (No rate increase)
BACKGROUND:
On June 25, 2013, the Board of Supervisors approved Contract #26-883-22 with Rodney Chan, D.P.M., for the
provision of professional podiatry clinic services and surgical procedures at Contra Costa Regional Medical Center
and Contra Costa Health Centers (CCRMC), for the period from June 1, 2013 through May 31, 2015. At the request
of the County, Contractor has agreed to provide additional podiatry clinics, increasing from 22 to 26 clinics per
month. Approval of Contract Amendment Agreement #26-883-23 will allow the Contractor to provide additional
professional podiatry services at CCRMC through May 31, 2015.

If this amendment is not approved, patients requiring professional podiatry services at CCRMC will not have access
to Contractor’s services, which may result in a reduction in the levels of service to the community.
