Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#26-742-1 with God’s Grace Caring Home, Inc., a corporation, in an amount not to exceed $280,000, to provide
residential board and care services for Contra Costa Regional Medical Center (CCRMC) patients in the Patch
Program, for the period from April 1, 2014 through March 31, 2015.
FISCAL IMPACT:
This Contract is funded 100% by the County’s Patch Program Funds. (Rate Increase)
BACKGROUND:
The County’s Patch Program provides residential board and care for post medical, surgical and/or custodial care
patients who have been discharged from CCRMC and would otherwise not have appropriate follow up care.

BACKGROUND: (CONT'D)
On June 25, 2013 the Board of Supervisors approved Contract #26-742 with God’s Grace Caring Home, Inc. for the
provision of residential board and care services for CCRMC patients in the Patch Program for the period from April
1, 2013 through March 31, 2014.
Approval of Contract #26-742-1 will allow the Contractor to continue to provide residential board and care services
through March 31, 2015.
Due to a clerical error, the Utilization Review department thought they submitted a renewal but it didn't go through.
The County owes the contractor for April and May (a total approximately $46,000). God's Grace Caring Home, Inc.
is a small business Board and Care facility that has accepted six of our patients. The owner is having cash flow issues
due to the delay in the renewal of the contract and has had to pay her staff reduced pay. There's a possibility they will
return the patients to us if we cannot expedite payment.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, these post-surgery patients would not have access to Contractor’s services, which
may result in a reduction in the levels of follow-up care service to the community.

CLERK'S ADDENDUM
By unanimous vote, with all Supervisors present, the Board found that an emergency exists involving provision of
services in public health or safety and did add item C.175 to the Board's agenda. The need for action came to the
attention of staff after the agenda had been posted.
