Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #74-341-36(5) with Salvador Lopez, MFT, a self-employed individual, effective April 15,
2014, to amend Contract #74-341-36(4), to increase the payment limit by $22,000, from $120,000 to a new payment
limit of $142,000, with no change in the original term of July 1, 2013 through June 30, 2015.
FISCAL IMPACT:
This amendment is funded 50% by Federal Medi-Cal funds and 50% State General Fund. (No rate increase)
BACKGROUND:
On August 6, 2013, the Board of Supervisors approved Contract #74-341-36(4) with Salvador Lopez, MFT for the
provision of Medi-Cal specialty mental health services at East and West County mental health clinics, for the period
from July 1, 2013 through June 30, 2015.

BACKGROUND: (CONT'D)
the time of negotiations, the payment limit was based on target levels of utilization. However, the utilization during
the first year of the agreement was higher than originally anticipated.
Approval of Contract Amendment Agreement #74-341-36(5) will allow the Contractor to provide additional services
through June 30, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, patients requiring Medi-Cal specialty mental health services will not have access
to Contractor’s services, which may result in a reduction in the levels of service to the community.
