HomeMy WebLinkAboutMINUTES - 06032014 - C.148RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment Agreement #22-940-13 with Public Health Foundation Enterprises, Inc., a non-profit corporation, effective April 1, 2014, to amend Novation Contract #22-940-12, to increase the payment limit by $18,504, from $119,488 to a new payment limit of $137,992, with no change in the original term of July 1, 2013 through June 30, 2014, and increase the automatic extension payment limit by $4,626 to a new total of $34,498, and no change in the term of the automatic extension, through September 30, 2014. FISCAL IMPACT: This amendment is funded 20% Federal Title III-C (1) of the Older Americans Act of 1965 and 80% by Federal Title III-C (2) of the Older Americans Act of 1965, through the Employment and Human Services Department. BACKGROUND: On July 16, 2013, the Board of Supervisors approved Novation Contract #22-940-12 with Public Health Foundation Enterprises, Inc. for the period from July 1, 2013 through June 30, 2014, which included a three-month automatic extension through September 30, 2014, for the provision of consultation, technical assistance and coordination services for County’s Senior Nutrition Program. Approval of Contract Amendment Agreement #22-940-13 will allow the Contractor to provide additional consultation, technical assistance, including coordination services through June 30, 2014. CONSEQUENCE OF NEGATIVE ACTION: If this amendment is not approved, Contractor will not provide additional services requested by County’s Senior Nutrition Program.
