HomeMy WebLinkAboutMINUTES - 06032014 - C.138RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment Agreement #74-443-4 with Ujima Family Recovery Services, a non-profit corporation, effective April 1, 2014, to amend Novation Contract #74-443-3, to increase the payment limit by $214,250, from $444,000 to a new payment limit of $658,250, with no change in the original term of July 1, 2013 through June 30, 2014, and to increase the automatic extension payment limit by $28,500 from $222,000 to a new payment limit of $250,500, with no change in the term of the automatic extension, through December 31, 2014. FISCAL IMPACT: This Amendment is funded 100% by SAMHWorks. Approval of Contract Amendment Agreement #74-443-3 will allow the Contractor to provide additional services to Clients enrolled in the SAMHWorks program as well as program enhancements to include staff training and equipment, through June 30, 2014. CONSEQUENCE OF NEGATIVE ACTION: If this amendment is not approved, Contractor will not be paid for additional services provided to County’s clients in need of treatment, case management or SAMHWorks assessments.
