CONSIDER waiving the 180-day “sit out period” for Dr. Mitchell Applegate, Exempt Medical Staff Physician in the Health Services Department; 2. FIND that the appointment of Dr. Applegate is necessary to fill a critically needed position; 3. APPROVE and AUTHORIZE the hiring of retiree Dr. Applegate as a temporary County employee for the period of May 14, 2014 through May 31, 2015, as recommended by the Health Services Department Director. FISCAL IMPACT: If the request is granted, there will continue to be salary and employment tax payment for the hours provided of approximately $79,929.60. These costs will be offset by savings due to the vacancy of the permanent position. Dr. Applegate was hired as an Exempt Medical Staff Physician on July 1, 1985, with Health Services Department. He completed his training in Family Medicine here, left to do fellowship training in critical care, and then spent approximately twenty years as the Director of Intensive Care Unit at Contra Costa Regional Medical Center. Dr. Applegate is broadly trained and experienced in hospital based medicine, pulmonary, critical care, perioperative and geriatric medicine. Up to his retirement, he continued to maintain an active role providing patient care in Medical, Surgical and Intensive Units as well as Pulmonary Clinic as a teacher, a participant for process improvement work, and a mentor. Health Services needs to return Dr. Applegate immediately as all areas of his expertise are currently severely understaffed and require ongoing coverage. The need is particularly acute on weekends where traditionally patient access to these services is challenging. Health Services has been actively recruiting to fill various clinical roles served by Dr. Applegate. Recruitment efforts include the use of temporary physician agencies, head hunting firms, internal recruitment of our family medicine graduates, as well as extensive advertisements in medical journals and professional organizations. We request approval to hire Dr. Applegate as a retiree to work in the Intensive Care Unit as well as Medical and Surgical Units at Contra Costa Regional Medical Center. In addition, he will provide pulmonary consultation, and palliative care consultations on the weekends, which will maintain patient access and staff access to these resources. His loss would cause a significant negative impact in our ability to provide necessary critical care services. His twenty-five years of experience at Contra Costa Regional Medical Center will be very difficult to replace. While we continue to recruit and train staff to manage the areas of Dr. Applegate’s expertise, the organization depends on his skill set, training, and experience on an on-going basis. CONSEQUENCE OF NEGATIVE ACTION: Failure to receive Board approval will result in significant negative impact in our ability to provide necessary critical care services for our patients.
