Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-722-2 with Rose Susan Cohen, M.D. a self-employed individual, effective May 1, 2014,
to amend Contract #26-722-1 to modify the Payment Provisions, with no change in the payment limit of $1,350,000
or original term of May 1, 2013 through April 30, 2016.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. (No rate increase)
BACKGROUND:
On June 4, 2013, the Board of Supervisors approved Contract #26-722-1 with Rose Susan Cohen, M.D. for the
provision of professional cardiology services at Contra Costa Regional Medical Center and Health Centers
(CCRMC), for the period from May 1, 2013 through April 30, 2016.
At the request of the Division, Contractor is now expected to provide clinical and administrative support, as assigned
and approved

BACKGROUND: (CONT'D)
in advance by the Medical Director. This amendment modifies the payment provision to add $150 per hour, for
clinical and administrative support, with no change in the contract payment limit or term.
Approval of Contract Amendment Agreement #26-722-2 will allow the Contractor to provide additional services to
include clinical and administrative support as assigned by the Medical Director at CCRMC through April 30, 2016.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract amendment is not approved, Contractor will not be expected to provide administrative or clinical
support as directed by the Medical Director.
