Hayashi, M.D., Inc., a professional corporation, in an amount not to exceed $1,800,000, for the provision of professional radiology services for patients at Contra Costa Regional Medical Center and Contra Costa Health Centers (CCRMC), for the period from July 1, 2014 through June 30, 2017. FISCAL IMPACT: This Contract is funded 100% by Enterprise Fund I. (No rate increase) BACKGROUND: On June 18, 2011, the Board of Supervisors approved Contract #26-445-5 (as amended by Amendment Agreements #26-445-6, #26-445-7, #26-445-8, and #26-445-9) with Aaron K. Hayashi, M.D., Inc. BACKGROUND: (CONT'D) and interpretation of CT Scans, MRIs, Ultrasounds, invasive procedures and plain films for CCRMC. Approval of Contract #26-445-10 will allow the Contractor to continue to provide professional radiology services at CCRMC, through June 30, 2017. CONSEQUENCE OF NEGATIVE ACTION: If this contract is not approved, patients requiring professional radiology services at CCRMC will not have access to Contractor’s services, which may result in a reduction in the levels of service to the community.
