SUBJECT; Approval of Standard Agreement #29-315-31; with the State Department of Health
Services (State #86-88668) for Preventive Health Care for the Aging
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chairman to execute on behalf of the County, Standard

Agreement #29-315-3 (State #86-88668) with the State Department of Health Services
in the amount of $45,000 for the period July 1, 1986 - June 30, 1987 for the
PreventiveHealth Care for the Aging Program.'„
II. FINANCIAL IMPACT: j
Approval of this agreement by the State will result in $45,000 of State funding for
preventive health care for the aging. Sourcesll of funding are as follows:

The County received $45,000 of funding from the State for this program last fiscal
year . The County share of costs for this program are included in the FY 1986-87
budget.

III. REASONS FOR RECOMMENDATIONS/BACKGROUND: l
On March 4, 1986, the Board authorized submission of a funding application to the
State Department of Health Services for continuation of the Preventive Health Care
for the Aging Program, also known as the Healthy Older Adult Program (HOAP). This
program provides preventive health education and health promotion activities to the
elderly in order to help senior citizens maintain or improve their health through
health problem identification, referral for problems found, health counseling and
education.
This document has been approved by the '; Department's Contracts and Grants
Administrator in accordance with the guidelines approved by the Board's Order of
December 1, 1981 (Guidelines for contract 'I, preparation and processing, Health
Services Department) .

The Board Chairman should sign eight copies of the contract, seven of which should
then be returned to the Contracts and Grunts Unit for submission to State
Department of Health Services.
