Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment/Extension Agreement #27-633-10 with PerformRx, LLC, a limited liability company, effective April 30,
2014, to amend Contract #27-633-9, to increase the payment limit by $55,000,000, from $42,000,000, to a new
payment limit of $97,000,000, and extend the term from April 30, 2014 through April 30, 2015.
FISCAL IMPACT:
This Amendment is funded 100% by Contra Costa Health Plan member premiums. (No rate increase)
BACKGROUND:
On April 9, 2013, the Board of Supervisors approved Contract #27-633-9, with PerformRx, LLC for the provision of
pharmacy administration services for the Contra Costa Health Plan, for the period from May 1, 2013 through April
30, 2014.
Approval of Contract Amendment/Extension Agreement #27-633-10 will allow the Contractor to continue providing
pharmacy administration services through April 30, 2015, including continuation of agreeing to a mutual
indemnification clause to hold each other harmless for
