Approve and Authorize the Health Services Director or his designee, to execute, as follows:
A. Agreement #28-789 (State #13-9014) with the State of California Health Benefit Exchange, dba Covered
California, the Exchange” a government agency, for the County to act as a Certified Enrollment Entity for the
In-Person Assistance Program (IPA), for the period from July 1, 2013 through December 31, 2014; and
B. Amendment Agreement 28-789-1 (State #13-9014 Addendum 1) with the State of California Health Benefit
Exchange (dba Cover California, the “Exchange”) , effective July 26, 2013, to make technical adjustments to the
agreement, with no change in the original term of July 1, 2013, through December 31, 2014.
FISCAL IMPACT:
Approval of the Agreement will result in a non-financial agreement to allow the County to provide access to Covered
Services to Enrollees in the benefits exchange Qualified Health Plan (QHP) program. No County match required.

Subject:Agreement #29-789 and Amendment Agreement #29-789-1 with the State of California Health Benefit Exchange
BACKGROUND:
The mission of the California Health Benefit Exchange (Exchange”) and Contra Costa Health Plan, (Qualified Health
Plan, QHP) is to increase the number of insured Californians, improve health care quality and access to care, promote
health, lower costs, and reduce health disparities. The Exchange seeks to accomplish its mission by creating an
innovative, competitive marketplace that empowers consumers to choose the health plan and providers that offer the
best value. The Exchanges’ “triple aim” framework also seeks to improve the patient care experience including
quality and satisfaction, improve the health of the population and reduce the per capita cost of Covered Services.
Approval of this Agreement #29-789 and Amendment #29-789-1 will allow County to provide access to Covered
Services to Enrollees regarding delivery of services and benefits to Enrollees, coordinate and cooperate with the
Exchange on the promotion of better care and higher value of Enrollees and other health care consumers, and
enhance alignment between participating providers to deliver better care and higher value through December 31, 2014.
CONSEQUENCE OF NEGATIVE ACTION:
If this Agreement is not approved, the County will not be able to participate in the Exchange program to provide
covered access to Covered Services to Enrollees in Contra Costa County.
