HomeMy WebLinkAboutMINUTES - 04222014 - C.91RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment Agreement #26-710-3 with Janet Diaz, M.D., A Professional Corporation, effective May 1, 2014, to amend Contract #26-710-1 (as amended by Amendment Agreement #26-710-2), to increase the payment limit by $90,000, from $420,000 to a new payment limit of $510,000, with no change in the original term of November 14, 2012 through October 31, 2014. FISCAL IMPACT: This amendment is funded 100% by Enterprise Fund I. At the request of the Division, the Contractor has agreed to provide an increase in her workload and is projected to work more clinics and on-call shifts than originally anticipated. As a result, the Division is recommending that contract year two be amended to reflect the increase in Contractor’s services. Approval of Contract Amendment Agreement #26-710-3 will allow the Contractor to provide additional professional pulmonary services through October 31, 2014. CONSEQUENCE OF NEGATIVE ACTION: If this amendment is not approved, patients requiring professional pulmonary services at CCRMC will not have access to Contractor’s services, which may result in a reduction in the levels of service to the community.
