Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-445-9 with Aaron K. Hayashi, M.D., Inc., a professional corporation, effective April 1,
2014, to amend Contract #26-445-5 (as amended by Amendment Agreements #26-445-6, #26-445-7, and #26-445-8),
to increase the payment limit by $75,000, from $1,600,000 to a new payment limit of $1,675,000, with no change in
the original term of July 1, 2011 through June 30, 2014.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. (No rate increase)
BACKGROUND:
On June 28, 2011, the Board of Supervisors approved Contract #26-445-5 (as amended by Amendment Agreements
#26-445-6, #26-445-7, and #26-445-8) with Aaron K. Hayashi, M.D., Inc. for the provision of professional radiology

BACKGROUND: (CONT'D)
services at Contra Costa Regional Medical Center (CCRMC), for the period from July 1, 2011 through June 30, 2014.
At the request of the Division, Contractor has taken on extra days and additional on-call shifts due to an increase in
staffing cut backs. The Division is requesting a payment limit increase to cover Contractor’s additional service
through the remainder of this contract period.
Approval of Contract Amendment Agreement #26-445-9 will allow the Contractor to provide additional professional
radiology services at CCRMC through June 30, 2014
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, patients requiring professional radiology services at CCRMC will not have access
to Contractor’s services, which may result in a reduction in the levels of service to the community.
