Approve and authorize the Health Services Director, or his designee, to execute, on behalf of County, Grant
Amendment/Extension Agreement #28-349-1 with the Opticos Design, Inc., effective July 1, 2013 to amend Grant
Agreement #28-349, with no change in the original amount of $70,000 payable to County, to extend the term from
July 1, 2013 through June 30, 2014.
FISCAL IMPACT:
Approval of this amendment will result in no change in the original funding of $70,000 from Opticos Design, Inc. for
the Public Health City of Richmond Livable Corridors – Health Impact Assessment Project. No County match
required.
BACKGROUND:
On February 14, 2012, the Board of Supervisors approved Grant Award Agreement #28-349 with the Opticos
Design, Inc., for the County’s Public Health City of Richmond Livable Corridors – Health Impact Assessment

BACKGROUND: (CONT'D)
July 1, 2013, of which included mutual indemnification to hold harmless both parties for any claims arising out of the
performance of this Agreement.
In collaboration with the City of Richmond, Opticos Design, Inc. and County’s Community Wellness and Prevention
Program (CW&PP), the City of Richmond’s Livable Corridors Project guided the transformation and revitalization
of major commercial corridors within the central urban core of the city into attractive, walkable destinations.
Approval of Grant Amendment Agreement #28-349-1, will extend the term of the agreement, allowing continuation
of the County’s Public Health City of Richmond Livable Corridors – Health Impact Assessment Project through June
30, 2014.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, the County will not be able to continue the City of Richmond’s Livable Corridors
Project.
