Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#25–012–29 with The Center for Common Concerns, Inc. (dba HomeBase), a non-profit corporation, in an amount
not to exceed $296,950, to provide consultation and technical assistance to the Department with regard to the
Continuum of Care planning and resource development, for the period from October 1, 2013 through September 30,
2014.
FISCAL IMPACT:
This Contract is funded 7% by Federal Medi-Cal Administrative Activities (MAA), 37% by budgeted County
General funds and 56% by Medi-Cal Administrative Activities/HUD. (No rate increase)
BACKGROUND:
On January 22, 2013, the Board of Supervisors approved Contract #25–012–26, (as amended by Amendment
Agreements #25-012-27 through #25-012-28) with The Center for Common Concerns, Inc. (dba HomeBase), for
the period from October 1, 2012 through September 30, 2013, for the provision of consultation and technical
assistance to the Department with regard to the Continuum of Care planning and resource development, including
grant-writing services for County’s McKinney-Vento application.

BACKGROUND: (CONT'D)
Approval of Contract #25–012–29 will allow the Contractor to continue providing services through September 30,
2014.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, County will no longer have this Contractor's expertise needed to meet all federal
guidelines to secure maximum McKinney-Vento funding required to implement the 10-year plan to eliminate
homelessness in Contra Costa County.
