Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Grant
Agreement #29-543 with Mt. Diablo Health Care District, a government agency, to pay the County an amount not to
exceed $11,350, for the operation of Philip Dorn Respite Center in the City of Concord, for the period from August 1,
2013, through June 30, 2014.
FISCAL IMPACT:
Approval of this Agreement will result in an amount not to exceed $11,350 payable to the County from Mt Diablo
Health Care District for the Philip Dorn Respite Center. No County match required.
BACKGROUND:
The County Homeless program is seeking funding through the Mt. Diablo Health Care District to support the
operation of the Philip Dorn Respite Center. The goal of the center is to provide outreach, short-term shelter,
goal-oriented counseling, and health services to medically fragile homeless individuals living on the streets of Contra
Costa County or recently discharged

BACKGROUND: (CONT'D)
from hospitals. The Philip Dorn Respite Center is a 24 bed interim housing program, for homeless individuals, who
required recuperative health care services, but who no longer meet criteria for hospitalization.
Approval of Grant Agreement #29-543 will allow the County to receive funds to support the Philip Dorn Respite
Center, including agreeing to indemnify and hold the grantor harmless for claims arising out of the County’s
performance under the agreement through June 30, 2014.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, homeless individuals who are medically vulnerable and require a higher level of
medical care will not receive services at this homeless shelter facility.
