Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#26-672-2 with Siri Sunderi Cheng, M.D. a self-employed individual, in an amount not to exceed $1,065,000, to
provide professional otolaryngology services at Contra Costa Regional Medical Center and Contra Costa Health
Centers (CCRMC), for the period from October 1, 2013 through September 30, 2016
FISCAL IMPACT:
This Contract is funded 100% by Enterprise Fund I. Cost to the County depends on utilization. As appropriate,
patients and/or third party payors will be billed for services. (Rate increase)
BACKGROUND:
For a number of years the County has contracted with Medical, Dental, and Mental Health Specialists to provide
specialized professional services that are not otherwise available in its Hospital and Health Centers.
On September 28, 2010, the Board of Supervisors approved Contract #26-672 (as amended by

BACKGROUND: (CONT'D)
Amendment Agreement #26-672-1) with Siri Sunderi Cheng, M.D. for the provision of professional otolaryngology
services including consultation, training, administration services, on call coverage and medical/surgical procedures at
CCRMC, for the period from October 1, 2010 through September 30, 2013.
Approval of Contract #26-672-2 will allow Contractor to continue providing professional otolaryngology services
through September 30, 2016.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, patients requiring professional otolaryngology services at CCRMC will not have
access to Contractor’s services, which may result in a reduction in the overall levels of services to the community.
