Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Extension Agreement #72-023-1 with Carlos Tejada (dba Tamalpais Vending Company), a self-employed individual,
to extend the term of the Contract from September 30, 2013 through October 31, 2013.
FISCAL IMPACT:
None, there is no change in the Contract Payment Limit of $0. County receives 20% commission on total gross
vending sales.
BACKGROUND:
On September 27, 2011, the Board of Supervisors approved Contract #72-023 with Carlos Tejada (dba Tamalpais
Vending Company), for the provision of healthy vending machine services, for the period from October 1, 2011
through September 30, 2013.
County and Contractor has agreed to extend the term of the Contract for one month to allow Contractor to remove
food prodcuts and vendor machines from selected

CONSEQUENCE OF NEGATIVE ACTION:
If this extension is not approved, County will not be compensated for vending machine sales provided by Contractor,
as mutually agreed, through the extended term of the Contract.
