APPROVE the Third Amendment to Lease with Thalapaneni Family Trust to extend the term from January 1, 2013 to
October 31, 2016, for approximately 8,050 square feet of office space located at 3501 Lone Tree Way, Suites 2 & 4,
Antioch, for continued occupancy by the Health Services Department, under the terms and conditions set forth in the
Lease.
AUTHORIZE the Public Works Director, or designee, to execute the Third Amendment to Lease on behalf of the
County and to exercise any options to extend the Lease.
FISCAL IMPACT:
The Lease will obligate the County to pay a total rent of $565,116 during the extended term. The amount was
anticipated and budgeted in the Health Services Department’s approved FY 2013/14 budget. (100% Health Services
Department-Children’s Mental Health; General Fund; Org# 5954)

Subject:3501 Lone Tree Way, Suites 2 & 4, Antioch - Lease for Health Services Department
BACKGROUND:
The County has leased this space for occupancy by the Health Services Department – Emergency Medical
Services since December 2008. The County is in the process of looking for another location for the Children’s
Mental Health program to relocate. The Third Amendment to Lease includes the County’s right to terminate the
Lease beginning March 1, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
Failure to approve the Lease may result in having to find new space for the Health Services Department –
Children’s Mental Health program, which could result in higher costs to the County.
CHILDREN'S IMPACT STATEMENT:
Not applicable.
