Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#27-914 with Tom H. Piatt, M.D., Inc., a corporation, in an amount not to exceed $100,000, to provide Diagnostic
Imaging to Contra Costa Health Plan members for the period from September 1, 2013 through August 31, 2015.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan Member Premiums.
BACKGROUND:
For a number of years the County has contracted with Medical, Dental and Mental Health Specialists to provide
specialized professional services, which are not otherwise available.
Under Contract #27-914, the Contractor will provide Diagnostic Imaging services to Contra Costa Health Plan
members through August 31, 2015.
