Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #25-012-28 with The Center for Common Concerns, Inc. (dba HomeBase), a non-profit
corporation, effective August 1, 2013, to amend Contract #25-012-26 (as amended by Contract Amendment
Agreement #25-012-27), to increase the payment limit by $30,000, from $301,850 to a new payment limit of
$331,850, with no change in the original term of October 1, 2012 through September 30, 2013.
FISCAL IMPACT:
This amendment is funded 6% Substance Abuse Mental Health Services Administration, 30% by Federal Medi-Cal
Administrative Activities (MAA), 9% Behavior Health Administration and 55% by budgeted County General funds.

BACKGROUND:
On January 22, 2013, the Board of Supervisors approved Contract #25-012-26 (as amended by Contract Amendment
Agreement #25-012-27) with The Center for Common Concerns, Inc. (dba HomeBase), for the provision of
consultation and technical assistance to the County’s Behavioral Health Services Division/homeless with regard to
development and implementation of the following:

BACKGROUND: (CONT'D)
1) Continuum of Care Implementation; 2) Continuum of Care Work Plan; Preparation of McKinney-Vento and
Substance Abuse and Mental Health Services Administration funding application; 3) Ten-year Plan to end Chronic
Homelessness; 4) Concord Naval Weapons Station Based conversion consultation; and 5) Behavioral Health Court
Coordination, including Grant writing services, for the period from October 1, 2012 through September 30, 2013.
Approval of Contract Amendment Agreement #25-012-28 will allow the Contractor to provide additional consulting
services to continue the Behavioral Health Services Division integration Program Planning and design and
Behavioral Health Court Coordination, through September 30, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, County will not receive Contractors expertise with the federal guidelines to secure
maximum McKinney-Vento funding required to implement the 10-year plan to eliminate homelessness in Contra
Costa County.
