APPROVE the Second Amendment to Lease with Mt. Diablo Unified School District to extend the term for four
years from October 1, 2013 to September 30, 2017, for approximately 2,400 square feet of medical office space
located at 215 Pacifica Avenue, Bay Point, for continued occupancy by the Health Services Department, under the
terms and conditions set forth in the Lease. (WLP809)
AUTHORIZE the Public Works Director, or designee, to execute the Second Amendment to Lease on behalf of the
County and to exercise any options to extend the Lease.
AUTHORIZE the Auditor-Controller to issue a check in the sum of $4.00 made payable to Mt. Diablo Unified
School District for payment in full for the four-year extension term.
FISCAL IMPACT:
The Lease will obligate the County to pay rent of $1.00 per year during the four-year term. The amount was
anticipated and budgeted in the Health Services Department’s approved FY 2013/14 budget. (85% Hospital
Enterprise Fund; 15% General Fund)

Subject:215 Pacifica Avenue, Bay Point – Second Amendment to Lease for the Health Services Department
BACKGROUND:
The County has leased this space for use by the Health Services Department since May 2006 to provide health
services and education to the community. The Health Services Department requires this medical office space for
use by the Contra Costa Health Centers Division as they have no other locations in Bay Point for those services.
CONSEQUENCE OF NEGATIVE ACTION:
Failure to approve the Second Amendment to Lease may result in having to find new space for the Health Services
Department’s education and medical programs, which may result in higher costs to the County.
CHILDREN'S IMPACT STATEMENT:
Not applicable.
