Change Order to Purchase Order #F39234 with Acumed Medical Supplies, Inc. to add $50,000 for a new total of $149,000 for orthopedic products at Contra Costa Regional Medical Center (CCRMC), for the period from October 15, 2011 through October 14, 2013; and B. Purchase Order with Acumed Medical Supplies, Inc. in the amount of $200,000 for orthopedic products at CCRMC for the period of October 15, 2013 through October 14, 2015. FISCAL IMPACT: 100% funding is included in the Enterprise Fund I Budget. These items are essential for performing orthopedic surgeries at CCRMC. CONSEQUENCE OF NEGATIVE ACTION: If this Purchase Order is not approved orthopedic surgeries cannot be performed at CCRMC. CHILDREN'S IMPACT STATEMENT: Not applicable.
