Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #23-312-10 with La Clinica de La Raza, Inc., a non-profit corporation, effective June 1,
2013, to amend Contract #23-312-8 (as amended by Contract Amendment/Extension Agreement #23-312-9), to
increase the payment limit by $200,000, from $350,000 to a new payment limit of $550,000, with no change in the
original term of May 1, 2012 through April 30, 2014.
FISCAL IMPACT:
This amendment is funded 100% from the General Fund. (No rate increase)
BACKGROUND:
On May 15, 2012, the Board of Supervisors approved Contract #23-312-8 (as amended by Contract
Amendment/Extension Agreement #23-312-9) with La Clinica de La Raza, Inc. for the period from May 1, 2012
through April 30, 2014, for the provision of primary medical care services to low income uninsured residents of
Contra Costa County with modifications to the General Conditions from the standard County format.

BACKGROUND: (CONT'D)
of Contract Amendment Agreement #23-312-10 will allow the Contractor to provide additional primary medical care
services through April 30, 2014.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, payment to Contractor for services will not be approved and patients will
experience longer wait times and potentially go untreated, due to reduced access to the medical services provided at
Contractor’s clinic.
