HomeMy WebLinkAboutMINUTES - 08132013 - C.25RECOMMENDATION(S): ADOPT Resolution No. 21346 to add two (2) 40/40 Community Health Worker II positions (VKVB) ($2,918.46 - $3,547.41) in the Health Services Department. FISCAL IMPACT: The additional annual cost associated with this action will be $138,286.62 annually with benefits. The cost of this action will be offset by the New Access Point (NAP) grant and fees. BACKGROUND: The New Access Point (NAP) grant administered by HRSA provides $649,185 per year in additional staff funding to the Health Care for the Homeless Program. Health Services will use this funding to staff Community Health Workers to function as part of a team at the new Concord Health Center Building #2. This team will be completely integrated into the ambulatory care clinic to provide a higher level of comprehensive care to all patients. Services will include primary medical care, mental health services, substance abuse treatment and oral health care.
