Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #74-317-8 with Alternative Family Services, Inc., a non-profit corporation, effective April 1,
2013, to amend Novation Contract #74-317-7, to increase the payment limit by $122,437, from $600,000 to a new
payment limit of $722,437, with no change in the original term of July 1, 2012 through June 30, 2013 , and to
increase the automatic extension payment limit by $61,219, from $300,000 to a new payment limit of $361,219, with
no change in the term of the automatic extension, through December 31, 2013.
FISCAL IMPACT:
This amendment is funded 50% by Federal Financial Participation and 50% by County Mental Health Realignment.
(No rate increase)
BACKGROUND:
This Contract meets the social needs of County’s population in that it provides Multidimensional Treatment Foster
Care (MTFC) services to Severely Emotionally Disturbed (SED) youth who are in foster care or in Intensive
Treatment Foster Care homes, and their families, including mental health services, medication support, and crisis
intervention.

BACKGROUND: (CONT'D)
On January 8, 2013, the Board of Supervisors approved Novation Contract #74-317-7 with Alternative Family
Services, Inc. for the period from July 1, 2012 through June 30, 2013, which included a six-month automatic
extension through December 31, 2013, for the provision of a MTFC program for SED adolescents.
At the time of negotiations, the payment limit was based on target levels of utilization. However, the utilization
during the term of the agreement was higher than originally anticipated. Approval of Contract Amendment
Agreement #74-317-8 will allow the Contractor to provide additional services through June 30, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, Contractor will not be paid for its MTFC services provided in good faith to
County clients.
CHILDREN'S IMPACT STATEMENT:
This program supports the following Board of Supervisors’ community outcomes: “Children Ready For and
Succeeding in School”; “Families that are Safe, Stable, and Nurturing”; and “Communities that are Safe and Provide
a High Quality of Life for Children and Families”. Expected program outcomes include an increase in positive social
and emotional development as measured by the Child and Adolescent Functional Assessment Scale (CAFAS).
