Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #27-800-3 with VVS Primary Care Medical Group, Inc. (dba Piedmont Primary Care) , a
corporation, effective June 1, 2013, to amend Contract #27-800-1, to increase the payment limit by $150,000, from
$50,000 to a new payment limit of $200,000, with no change in the original term of May 1, 2012 through April 30,
2014.
FISCAL IMPACT:
This amendment is funded 100% by Member Premiums. (No rate increase)
BACKGROUND:
The Health Plan has an obligation to provide certain specialized professional health care services for its members
under the terms of their Individual and Group Health Plan membership contracts with the County.
In May 2012, the County Administrator approved and Purchasing Services Manager executed Contract #27-800-1,
with Clinica la Luna y el Sol Medical Center, Inc. [Now known as VVS Primary Care Medical

provision of Primary Care services for Contra Costa Health Plan members.
Approval of Contract Amendment Agreement #27-800-3 will allow the Contractor to provide additional Primary
Care services through April 30, 2014.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, certain specialized professional health care services for its members under the
terms of their Individual and Group Health plan membership contracts with the County will not be provided.
