Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#74-341-34(3) with Ellen Ayres, LCSW, a self-employed individual, in an amount not to exceed $112,000, to provide
Medi-Cal specialty mental health services for the period from July 1, 2013 through June 30, 2015.
FISCAL IMPACT:
This Contract is funded 100% by Medi-Cal Funds offset 50% State and 50% Federal (No rate increase)
BACKGROUND:
In July 2011, the County Administrator approved and Purchasing Services Manager executed Contract
#74-341-34(1), [as amended by Contract Amendment Agreement #74-341-34(2)], with Ellen Ayres, LCSW, for the
period from July 1, 2011 through June 30, 2013 for the provision of Medi-Cal specialty mental health services.
Approval of Contract #74-341-34(3) will allow Contractor to continue providing Medi-Cal specialty mental health
services through June 30, 2015.
