Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #74-341-30(5) with Karen Benjamin, LCSW, a self-employed individual, effective
November 1, 2012 , to amend Contract #74-341-30-2 (as amended by Amendment Agreement #74-341-30(4), to
increase the payment limit by $54,000, from $100,000 to a new payment limit of $154,000, with no change in the
original term of July 1, 2011 through June 30, 2013.
FISCAL IMPACT:
This amendment is funded 100% by Medi-Cal funds off set 50% State and 50% Federal. (No rate increase)
BACKGROUND:
In May 2011, the Board of Supervisors approved Contract #74-341-30(2) as amended by Amendment Agreement
#74-341-30(4) with Karen Benjamin, LCSW for the period July 1, 2011through June 30, 2013 for the provision of
Medi-Cal specialty mental health services. At the time of negotiations, the payment limit was based on target levels
of utilization. However, the utilization

Approval of Contract Amendment Agreement #74-341-30(5) will allow the Contractor to provide additional
Medi-Cal specialty mental health services through June 30, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, County’s clients will not have access to Contractor’s professional mental health
services, reducing overall levels of service to the community.
