Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-977-14 with Diablo Valley Oncology and Hematology Medical Group, Inc., a
corporation, effective November 1, 2012, to amend Contract #26-977-13, to modify and correct payment provisions
and Service Plan with no change in original Payment Limit of $300,000 and with no change in the term of November
1, 2012 through October 31, 2015.
FISCAL IMPACT:
Funded 100% by Enterprise Fund I. This Amendment will modify and correct the payment provisions to reflect the
rate of $300 per report for peer review, correct an error in the Medicare Physician’s Resource-Based Relative Value
Scale (RBRVS) and modify the Service Plan to define on-call coverage.
BACKGROUND:
On January 15, 2013, the Board of Supervisors approved Contract #26-977-13 with Diablo Valley Oncology and
Hematology Medical Group, Inc. for the period from November 1, 2012 through October 31, 2015, for the provision
of urology services to patients at Contra Costa Regional

BACKGROUND: (CONT'D)
Medical Center and Contra Costa Health Centers (CCRMC) including, but not limited to, clinic coverage,
consultation, training, on-call coverage, ultra sound, evaluation and management services, and medical and/or
surgical procedures
Approval of Contract Amendment Agreement #26-977-14 modifies and corrects the payment provisions of Contract
#26-977-13 and defines on-call coverage, which definition was inadvertently omitted from Contract #26-977-13.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, Contractor will not receive the compensation intended and agreed to by the parties
for urology services provided at Contra Costa Regional Medical Center and Contra Costa Health Centers.
