Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#22–806–21 with Public Health Foundation Enterprises, Inc., a non-profit corporation, in an amount not to exceed
$903,868, to provide consultation and technical assistance with regard to community health promotion for the
Partnership for Public Health (PPH) Program, for the period from July 1, 2013 through June 30, 2014.
FISCAL IMPACT:
This Contract is funded 58% by Federal Bioterrorism Grant, 6% by Public Health Laboratory, 8% by Health Care
Interpreter Network Reimbursement, and 28% by budgeted County funds. (No rate increase)
BACKGROUND:
This Contract meets the social needs of County’s population by providing organization services for the West County
Health Collaborative, Monument Community Partnership and Bay Point Community Partnership Programs. Services
provided by this Contract also include leadership and public health advocacy training and participation with PPH
Program’s Action Plans.

BACKGROUND: (CONT'D)
Supervisors approved Contract #22–806–20 with Public Health Foundation Enterprises, Inc., for the period from July
1, 2012 through June 30, 2013, for the provision of consultation and technical assistance with regard to community
health promotion for the PPH Program.
Approval of Contract #22–806–21 will allow the Contractor to continue providing services through June 30, 2014.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, County residents who depend on services from West County Health Collaborative,
Monument Community Partnership, and Bay Point Community Partnership Programs may not receive the services
they need.
