Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #74-443-2 with Ujima Family Recovery Services, a non-profit corporation, effective April
15, 2013, to amend Novation Contract #74-443 (as amended by Amendment Agreement #74-443-1), to increase the
payment limit by $55,500, from $820,031 to a new payment limit of $875,531, with no change in the original term of
March 1, 2012 through June 30, 2013, with no change in the automatic extension payment limit of $357,000, and no
change in the term of the automatic extension, through December 31, 2013.
FISCAL IMPACT:
This Amendment is funded 60% by CalWORKs Alcohol and Other Drug Services (AODS), 29% CalWORKs Mental
Health and 11% Substance Abuse Prevention and Treatment Federal Block Grant. (No rate increase)
BACKGROUND:
On June 5, 2012, the Board of Supervisors approved Novation Contract #74-443 (as amended by Amendment
Agreement #74-443-1) with Ujima Family Recovery Services for the period from March 1, 2012 through June 30,
2013, which included a six-month automatic extension through

BACKGROUND: (CONT'D)
December 31, 2013, for the provision of a perinatal day treatment program, as well as CalWORKs assessment
services.
Approval of Contract Amendment Agreement #74-443-2 will allow the Contractor to enhance its facility to provide
additional services to Clients enrolled in the CalWORKs program including, relocation, training and equipment,
through June 30, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, Contractor will not be paid for additional services provided to County’s clients in
need of perinatal day treatment, counseling or CalWORKs assessments.
