Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#27-272-9 with East Bay Retina Consultants A Medical Group, Inc., a corporation, in an amount not to exceed
$110,000, to provide professional ophthalmology and retina surgery services for the period from June 1, 2013
through May 31, 2015.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan (Health Plan) member premiums. Costs depend upon
utilization. (No rate increase)
BACKGROUND:
The Health Plan has an obligation to provide certain specialized professional health care services for its members
under the terms of their Individual and Group Health Plan membership contracts with the County.
In July, 2011, the County Administrator approved and Purchasing Services Manager executed Contract #27-292-8
with East Bay Retina Consultants A Medical Group, Inc., for the period from June 1, 2011 through May 31, 2013,
for the provision of professional ophthalmology and retina surgery services.

Approval of Contract #27-272-9 will allow Contractor to continue providing professional ophthalmology and retina
surgery services through May 31, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, certain specialized professional health care services for its members under the terms
of their Individual and Group Health Plan membership contracts with the County will not be provided.
