Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Agreement
#28-344-2 with the California Department of Health Care Services, to pay the County in an amount not to exceed
$50,000,000, for Targeted Case Management Services, for the period from July 1, 2013 through June 30, 2018.
FISCAL IMPACT:
Approval of this Agreement will result in an amount not to exceed $50,000,000 in funds payable to County for the
Targeted Case Management Services, through June 30, 2018, from the California Department of Health Care
Services. No additional County match is required.
BACKGROUND:
Since 1997, the Department of Health Care Services has funded many Local Governmental Agencies (LGA) to
provide Targeted Case Management (TCM) Services. Targeted Case Management (TCM) is a Federal Program
funding the provisions of comprehensive case management services to individuals within a specified target group.
Due to California’s “Bridge to Reform”, Section 1115 Medicaid demonstration waiver and the related Medi-Cal
managed care expansion, new

BACKGROUND: (CONT'D)
TCM policies and procedures require an amendment to the current TCM provider Participation Agreement.
Approval of Agreement #28-344-2 allows the County to continue receiving funding for Targeted Case Management
Services through June 30, 2018.
CONSEQUENCE OF NEGATIVE ACTION:
If this agreement is not approved, the County will not have funds accessible to continue the Targeted Case
Management Services.
