Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-938-19 with David H.C. Raphael, M.D., a self-employed individual, effective
December 1, 2012, to amend Contract #26-938-17, to increase the payment limit by $3,000, from $345,000 to a new
payment limit of $348,000, with no change in the original term of January 1, 2012 through December 31, 2012.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. (No rate increase)
BACKGROUND:
On January 10, 2012, the Board of Supervisors approved Contract #26-938-17 with David H.C. Raphael, M.D., for
the period from January 1, 2012 through December 31, 2012, for the provision of professional general surgery
services for patients at Contra Costa Regional Medical Center and Contra Costa Health Centers.
The original payment limit was based on target levels of utilization; however, the utilization during the term of the
agreement was higher than originally anticipated. Approval of Contract

Amendment Agreement #26-938-19 will allow the Contractor to be reimbursed for services provided through
December 31, 2012.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, Contractor will not receive compensation for services rendered at Contra Costa
Regional Medical Center and Contra Costa Health Centers in December 2012.
